The design known as outside-screw-and-yoke (OS&Y) is specifically engineered to prevent thread wetting, primarily due to its stem being positioned outside the valve. A yoke is incorporated into the design, consisting of two upward-extending arms emerging from the bonnet and converging to create a housing for the yoke nut (also referred to as stem nut, yoke bushing, or stem bushing). Unlike other stem designs, the hand wheel is affixed to the yoke nut as opposed to the stem. The yoke nut enjoys rotational freedom but remains secured at the top of the yoke by a yoke-nut retainer, which doubles as a bushing for the nut. The yoke nut possesses a left-hand internal thread that matches with the thread on the stem. To close the valve, the stem thread is deliberately left-handed, meaning turning the hand wheel in a clockwise direction results in the downward movement of the stem. This design ensures that the stem remains stationary and can even be pinned to the gate, effortlessly revealing the position of the gate within the valve simply by observing the top of the stem. The OS&Y design is universally employed in all sizes of cast iron and steel gate valves.

The stem design of valves features a deformable non-metallic packing that prevents any external leakage along the stem. This packing is situated inside the packing chamber or stuffin box, which is circular in shape and located in the bonnet. In ISRS and NRS stem designs, the packing is secured in place by a hollow packing nut and a tubular gland that thread onto the top of the bonnet.
However, in the OS&Y stem design, the packing arrangement is more intricate and involves a gland that is held in place by a gland flange. The gland flange is further restrained by a pair of gland bolts and nuts, which are fastened to the bonnet using eyes and pins or a similar arrangement. These measures ensure that the packing remains securely in place, preventing any leakage along the stem.
